End User License Agreement

1. Acceptance
By downloading or using MindCare by Dr. L, you agree to this End-User License Agreement (EULA).

2. License Grant
You are granted a limited, non-transferable license to use the app for education only. All content
remains the intellectual property of Julia Lesselyong, PsyD and MindCare by Dr. L.

3. Restrictions
You may not copy, modify, distribute, or resell the app. You may not use it for clinical treatment,
diagnosis, or crisis care. No PHI may be entered or stored.

4. Disclaimer
The app is for education only, not medical treatment. No guarantees or warranties are made. Use does
not establish a provider–patient relationship.

5. Privacy
No PHI is collected. Limited personal data may be collected for account and certification purposes. See
HIPAA statement for details.

6. Termination
MindCare by Dr. L may suspend or terminate access if terms are violated.

7. Governing Law
This EULA is governed by the laws of Arizona.
